AZHM is committed to transparency regarding payments and services. Please review the following policy carefully before scheduling your telehealth appointment.

Role of AZHM

AZHM operates as a marketing and patient acquisition platform. Medical services, including consultations, evaluations, and treatments, are provided by licensed healthcare professionals through a third-party telehealth partner.

  • AZHM does not provide medical care
  • AZHM does not process payments for medical services
  • All payments are handled by the telehealth partner platform

Payment Policy

Payments for telehealth services are collected through the telehealth partner platform. Pricing is clearly disclosed prior to scheduling.

By proceeding and initiating a transaction, users agree to the specific pricing and payment terms presented at the point of checkout.

Refund Policy

Refunds are only available if a patient does not qualify for treatment. All other payments are considered final and non-refundable.

Qualification Refund

If a licensed healthcare provider determines you are not eligible for treatment, you will receive a refund minus the consultation fee.

Non-Refundable After Qualification:

  • Consultation fees after qualification
  • Provider evaluation fees
  • Any associated service fees after treatment approval

By scheduling an appointment, you acknowledge and agree to this policy.

Cancellations and Rescheduling

Rescheduling

Appointments may be rescheduled in accordance with the telehealth provider's individual policies. Please refer to your confirmation email for instructions.

No-Shows

Missed appointments (no-shows) may not be eligible for rescheduling or credit. Policies regarding missed appointments vary depending on the provider.

Treatment Eligibility

Scheduling an appointment does not guarantee treatment approval. All treatment decisions are made solely by licensed healthcare providers based on their professional medical judgment.

If you are not approved for treatment, you will receive a refund minus the consultation fee.

Technical Issues

In the event of technical issues during a telehealth consultation, every effort will be made by the provider to reconnect or reschedule your session. Refund eligibility arising from technical failures is determined by the telehealth provider, not by AZHM.
